Accident and Damage Reports
The above Declaration of Surplus Property Form is to be used by departments who do not have access to TruView. Those with access to TruView must use the online version of the form. Instructions to complete the online form may be found by logging on to TruView, select the Business Office link, under the Forms section go to Inventory Control.
The completed Declaration of Surplus Property form should to be submitted to Inventory Control, Business Office, MC 105. You may send the completed form via email to firstname.lastname@example.org
