How to File a Complaint

Student Conduct Code Referral Instructions

If you believe that a student or student organization has acted in a way that violates the Student Conduct Code and wish to file a referral, please fill out the Referral Form.  The Referral Form will ask for the following information:

  1. Who:  The name of the accused student(s)/student organizations(s) is essential for investigating and charging the appropriate individuals/organizations involved in the alleged violation of the Student Conduct Code.
  2. When and Where:  The date and location of the incident and alleged violation of the Student Conduct Code assists in corroborating information in the case from multiple sources.  Any complaint should be submitted as soon as possible after the misconduct takes place or becomes known.
  3. What, Why, and How:  Description of alleged misconduct assists the University Conduct Officer in verifying the facts of the case and the sections(s) of the Student Conduct Code that are alleged to have been violated.
